Medication Review
If you have been asked to complete a medication review, this can be done online. Some medications require a blood test or blood pressure before we can re-authorise them. I have provided a list of the medications which require a blood test below. If you are on any of these medications, please arrange to have your bloods tested before filling in this form:

Once you have completed the form below, we will authorise everything from our end. If there is an issue we will email you back, or get in touch with you. We will not reply if we have managed to repeat your medications. If you find your medications have not been re-authorised, please get back in touch with us by phoning the practice.
