Join our practice
You can register as a patient by calling in at the surgery and completing a GMS1 form (below). It is helpful if you have your NHS Medical Card with you as we require your NHS number (available from your previous surgery if you do not have an NHS Medical card), we also require evidence of identity. You will be asked to complete a new patient form, which will give us a little more information in respect of your health until your records to arrive from your previous doctor. You will also be booked to see the nurse for a New Patient Check.
Please also complete the new patient questionnaire below, and bring it to the surgery with your GMS 1 form.
Although registration is with the practice and not an individual doctor, you may express a preference to see a particular doctor and your preference will be recorded.
